Support Coordination

St. John of God Community Services is proud to announce the launch of our newest service: the Support Coordination program tailored specifically for adults ages 21 and older with intellectual and developmental disabilities.

​

Support Coordination is a comprehensive case management service designed to empower adults with intellectual and developmental disabilities to navigate the complexities of various support systems and access the resources they need to thrive and lead meaningful lives. Our dedicated Support Coordinators work closely with participants and their families to develop personalized plans that address individual goals, preferences, challenges, and needs throughout the course of their lives.

What’s Included

Person-Centered Approach: Each individual is at the forefront of their care. Our Support Coordinators collaborate closely with individuals and families to identify their unique strengths, preferences, and aspirations, ensuring that services and supports align with their specific needs.

Resource Navigation: Navigating the array of available supports can be overwhelming. Our team provides expert guidance and assistance in accessing community resources, including healthcare, housing, vocational training, education, and social activities.

​

Advocacy: We are dedicated advocates for the rights and interests of individuals with special needs. Our Support Coordinators will work tirelessly to ensure that participants receive the services and supports to which they are entitled, advocating on their behalf with service providers, agencies, and other stakeholders.

​

Case Management: Our Support Coordinators serve as a central point of contact, facilitating communication and collaboration among service providers, healthcare professionals, and more to ensure seamless delivery of care. Our ultimate goal is to empower individuals to lead fulfilling and independent lives. Through personalized support plans, and goal-setting exercises, we equip participants with the tools and resources they need to achieve their aspirations and maximize their independence.
